Objectives To describe the safety of vaccines against SARS-CoV-2 in people with inflammatory/autoimmune rheumatic and musculoskeletal disease (I-RMD). Methods Physician-reported registry of I-RMD and non-inflammatory RMD (NI-RMDs) patients vaccinated against SARS-CoV-2. From 5 February 2021 to 27 July 2021, we collected data on demographics, vaccination, RMD diagnosis, disease activity, immunomodulatory/immunosuppressive treatments, flares, adverse events (AEs) and SARS-CoV-2 breakthrough infections. Data were analysed descriptively. Results The study included 5121 participants from 30 countries, 90% with I-RMDs (n=4604, 68% female, mean age 60.5 years) and 10% with NI-RMDs (n=517, 77% female, mean age 71.4). Inflammatory joint diseases (58%), connective tissue diseases (18%) and vasculitis (12%) were the most frequent diagnostic groups; 54% received conventional synthetic disease-modifying antirheumatic drugs (DMARDs), 42% biological DMARDs and 35% immunosuppressants. Most patients received the Pfizer/BioNTech vaccine (70%), 17% AstraZeneca/Oxford and 8% Moderna. In fully vaccinated cases, breakthrough infections were reported in 0.7% of I-RMD patients and 1.1% of NI-RMD patients. I-RMD flares were reported in 4.4% of cases (0.6% severe), 1.5% resulting in medication changes. AEs were reported in 37% of cases (37% I-RMD, 40% NI-RMD), serious AEs in 0.5% (0.4% I-RMD, 1.9% NI-RMD). Conclusion The safety profiles of SARS-CoV-2 vaccines in patients with I-RMD was reassuring and comparable with patients with NI-RMDs. The majority of patients tolerated their vaccination well with rare reports of I-RMD flare and very rare reports of serious AEs. These findings should provide reassurance to rheumatologists and vaccine recipients and promote confidence in SARS-CoV-2 vaccine safety in I-RMD patients. International audience

Using data from the Life in confinement (Vico) survey, this article analyses these disturbances by focusing on residential configurations, understood as the association between a dwelling (the space it provides, its location, its environment) and the composition of the household (considering all its occupants). The data allow to document the containment of spring 2020, as well as the continuation of the crisis until the end of 2020. By combining statistical descriptions and an analysis of the free comments written by the respondents, we show the importance of the “residential turbulence” (change of dwelling, hosting somebody, departure of cohabitants) and their links with biographical disturbances at the family and professional levels. The long-term nature of the crisis and the lack of temporal perspectives also lead people to reconsider their residential project and to readjust their temporal horizons in terms of housing, in anticipation of the crisis and its temporality, to adapt to the contingencies and uncertainties linked to the property market. Finally, the analysis of “residential turbulence” reveals the great diversity of the socio-economic profiles of individuals affected by the crisis without, however, neglecting the unequal dynamics that structure French society, as the crisis has reinforced situations of vulnerability experienced by poorly endowed individuals.; La epidemia de Covid-19 y las medidas adoptadas para contenerla han provocado una crisis que se refleja en la alteración de muchos cursos de la vida. A partir de los datos de la encuesta francesa Vida en Confinamiento (Vico), este artículo estudia dichas perturbaciones, centrándose en las configuraciones habitacionales, entendidas como la asociación entre una vivienda (el espacio que proporciona, su ubicación, su entorno) y la composición del hogar (considerando a todos sus ocupantes). Los datos permiten documentar el confinamiento de la primavera del 2020, así como el resto del periodo de crisis hasta finales de año. Combinando descripciones estadísticas y análisis de los comentarios libres escritos por los encuestados, mostramos la importancia de las “turbulencias habitacionales” (cambio de vivienda, acogida de personas, partida de convivientes) y sus vínculos con las perturbaciones biográficas a nivel familiar y profesional. El carácter duradero de la crisis y la falta de perspectivas temporales llevan a los individuos a reconsiderar su proyecto habitacional y a reajustar los horizontes temporales en materia de vivienda, según una lógica de anticipación de la crisis y de su temporalidad, con el fin de adaptarse a las contingencias e incertidumbres vinculadas al mercado inmobiliario. El análisis de las “turbulencias habitacionales” permite poner de manifiesto la gran diversidad de perfiles socioeconómicos de los individuos afectados por la crisis, sin dejar de lado las dinámicas inequitativas que estructuran la sociedad francesa. Porque, vale la pena recordarlo, la crisis refuerza las situaciones de vulnerabilidad que viven en particular las personas de bajos recursos.; L’épidémie de Covid-19 et les mesures prises pour l’endiguer ont provoqué une crise qui se traduit par la perturbation de beaucoup de parcours de vie. À partir des données de l’enquête La vie en confinement (Vico), cet article étudie ces perturbations en plaçant la focale sur les configurations résidentielles, entendues comme l’association entre un logement (l’espace qu’il procure, sa localisation, son environnement) et la composition du foyer (en considérant l’ensemble de ses occupants). Les données permettent de documenter le confinement du printemps 2020, ainsi que la suite de la crise jusqu’à la fin de l’année 2020. En combinant des descriptions statistiques et une analyse des commentaires libres rédigés par les personnes ayant répondu, nous montrons l’importance des « turbulences résidentielles » (changement de logement, accueil de personnes, départ de cohabitants) et leurs liens avec les perturbations biographiques sur les plans familial et professionnel. L’installation de la crise dans la durée et le manque de perspectives temporelles amènent également des individus à reconsidérer leur projet résidentiel et à réajuster les horizons temporels en matière de logement, selon une logique d’anticipation de la crise et de sa temporalité, pour s’adapter aux contingences et incertitudes liées au marché de l’immobilier. L’analyse des « turbulences résidentielles » permet de mettre à jour la grande diversité des profils socioéconomiques des individus touchés par la crise sans, toutefois, négliger les dynamiques inégalitaires qui structurent la société française, la crise venant renforcer des situations de vulnérabilité vécues par des individus faiblement dotés.

Le domaine de l'infectiologie constitue un champ de recherche transversal. Afin de l'identifier précisément, l'article construit un corpus global des publications en infectiologie en combinant les moyens offerts par le langage contrôlé du thésaurus MeSH de Medline et par la catégorisation des revues scientifiques du Web of Science. Ce corpus mondial permet de caractériser les publications des 20 premiers pays publiant dans le domaine et de retracer les évolutions entre 2000 à 2020. La construction de cartes thématiques permet d'identifier les thèmes de recherche au sein de l'infectiologie dans le monde et en France. L'explosion des publications sur le Covid-19 en 2020 a un impact très visible sur la carte mondiale des thématiques en infectiologie et modifie le positionnement de certains pays dans ce domaine de recherche. La conclusion identifie des pistes d'approfondissement de l'analyse du domaine qui pourraient être suivies à mesure que des données plus complètes seront disponibles sur la période de la pandémie de Covid-19. Research on infectious diseases constitutes a transversal scientific field. A specific corpus is designed by combining a controlled language (Medline MeSH thesaurus) and the categorization of journals (Web of Science). From this global corpus, the article characterizes the publications from the top 20 countries publishing in the field and evolutions between 2000 and 2020. Topic maps show the research themes within the field of infectious diseases both in the world and in France. The explosion of publications on Covid-19 in 2020 has a quite visible impact on the topic map in infectious diseases and changes the position of some countries in this field of research. The conclusion points to issues for further research as more complete data will become available on the Covid-19 period. International audience

Background: Coronavirus disease 2019 (COVID-19) emerged as a worldwide health crisis, overwhelming healthcare systems. Elevated cardiac troponin T (cTn T) at admission was associated with increased in-hospital mortality. However, data addressing the role of cTn T in major adverse cardiovascular events (MACE) in COVID-19 are scarce. Therefore, we assessed the role of baseline cTn T and cTn T kinetics for MACE and in-hospital mortality prediction in COVID-19. Methods: Three hundred and ten patients were included prospectively . One hundred and eight patients were excluded due to incomplete records. Patients were divided into three groups according to cTn T kinetics: ascending, descending, and constant. The cTn T slope was defined as the ratio of the cTn T change over time. The primary and secondary endpoints were MACE and in-hospital mortality. Results: Two hundred and two patients were included in the analysis (mean age 64.4 ± 16.7 years, 119 [58.9%] males). Mean duration of hospitalization was 14.0 ± 12.3 days. Sixty (29.7%) patients had MACE, and 40 (19.8%) patients died. Baseline cTn T predicted both endpoints ( p = 0.047, hazard ratio [HR] 1.805, 95% confidence interval [CI] 1.009 – 3.231 ; p = 0.009, HR 2.322, 95% CI 1.234 – 4.369) . Increased cTn T slope predicted mortality (p = 0.041, HR 1.006, 95% CI 1.000–1.011). Constant cTn T was associated with lower MACE and mortality (p = 0.000, HR 3.080, 95% CI 1.914–4.954, p = 0.000, HR 2.851, 95% CI 1.828–4.447). Conclusions: The present study emphasizes the additional role of cTn T testing in COVID-19 patients for risk stratification and improved diagnostic pathway and management.

Prior to the COVID-19 pandemic, a relative dearth of research existed in this domain regarding epidemics, which is surprising considering that this optimistic bias has been associated with a lack of engagement in protective behaviours critical in fighting twenty-first-century, emergent, infectious diseases. The current study addresses this gap in the literature by investigating whether people demonstrated optimism bias during the first wave of the COVID-19 pandemic in Europe, how this changed over time, and whether unrealistic optimism was negatively associated with protective measures. Taking advantage of a pre-existing international participative influenza surveillance network (n = 12,378), absolute and comparative unrealistic optimism were measured at three epidemic stages (pre-, early, peak), and across four countries—France, Italy, Switzerland and the United Kingdom. Despite differences in culture and health response, similar patterns were observed across all four countries. The prevalence of unrealistic optimism appears to be influenced by the particular epidemic context. Paradoxically, whereas absolute unrealistic optimism decreased over time, comparative unrealistic optimism increased, suggesting that whilst people became increasingly accurate in assessing their personal risk, they nonetheless overestimated that for others. Comparative unrealistic optimism was negatively associated with the adoption of protective behaviours, which is worrying, given that these preventive measures are critical in tackling the spread and health burden of COVID-19. It is hoped these findings will inspire further research into sociocognitive mechanisms involved in risk appraisal. International audience

Objectives To describe the safety of vaccines against SARS-CoV-2 in people with inflammatory/autoimmune rheumatic and musculoskeletal disease (I-RMD). Methods Physician-reported registry of I-RMD and non-inflammatory RMD (NI-RMDs) patients vaccinated against SARS-CoV-2. From 5 February 2021 to 27 July 2021, we collected data on demographics, vaccination, RMD diagnosis, disease activity, immunomodulatory/immunosuppressive treatments, flares, adverse events (AEs) and SARS-CoV-2 breakthrough infections. Data were analysed descriptively. Results The study included 5121 participants from 30 countries, 90% with I-RMDs (n=4604, 68% female, mean age 60.5 years) and 10% with NI-RMDs (n=517, 77% female, mean age 71.4). Inflammatory joint diseases (58%), connective tissue diseases (18%) and vasculitis (12%) were the most frequent diagnostic groups; 54% received conventional synthetic disease-modifying antirheumatic drugs (DMARDs), 42% biological DMARDs and 35% immunosuppressants. Most patients received the Pfizer/BioNTech vaccine (70%), 17% AstraZeneca/Oxford and 8% Moderna. In fully vaccinated cases, breakthrough infections were reported in 0.7% of I-RMD patients and 1.1% of NI-RMD patients. I-RMD flares were reported in 4.4% of cases (0.6% severe), 1.5% resulting in medication changes. AEs were reported in 37% of cases (37% I-RMD, 40% NI-RMD), serious AEs in 0.5% (0.4% I-RMD, 1.9% NI-RMD). Conclusion The safety profiles of SARS-CoV-2 vaccines in patients with I-RMD was reassuring and comparable with patients with NI-RMDs. The majority of patients tolerated their vaccination well with rare reports of I-RMD flare and very rare reports of serious AEs. These findings should provide reassurance to rheumatologists and vaccine recipients and promote confidence in SARS-CoV-2 vaccine safety in I-RMD patients. International audience

Using data from the Life in confinement (Vico) survey, this article analyses these disturbances by focusing on residential configurations, understood as the association between a dwelling (the space it provides, its location, its environment) and the composition of the household (considering all its occupants). The data allow to document the containment of spring 2020, as well as the continuation of the crisis until the end of 2020. By combining statistical descriptions and an analysis of the free comments written by the respondents, we show the importance of the “residential turbulence” (change of dwelling, hosting somebody, departure of cohabitants) and their links with biographical disturbances at the family and professional levels. The long-term nature of the crisis and the lack of temporal perspectives also lead people to reconsider their residential project and to readjust their temporal horizons in terms of housing, in anticipation of the crisis and its temporality, to adapt to the contingencies and uncertainties linked to the property market. Finally, the analysis of “residential turbulence” reveals the great diversity of the socio-economic profiles of individuals affected by the crisis without, however, neglecting the unequal dynamics that structure French society, as the crisis has reinforced situations of vulnerability experienced by poorly endowed individuals.; La epidemia de Covid-19 y las medidas adoptadas para contenerla han provocado una crisis que se refleja en la alteración de muchos cursos de la vida. A partir de los datos de la encuesta francesa Vida en Confinamiento (Vico), este artículo estudia dichas perturbaciones, centrándose en las configuraciones habitacionales, entendidas como la asociación entre una vivienda (el espacio que proporciona, su ubicación, su entorno) y la composición del hogar (considerando a todos sus ocupantes). Los datos permiten documentar el confinamiento de la primavera del 2020, así como el resto del periodo de crisis hasta finales de año. Combinando descripciones estadísticas y análisis de los comentarios libres escritos por los encuestados, mostramos la importancia de las “turbulencias habitacionales” (cambio de vivienda, acogida de personas, partida de convivientes) y sus vínculos con las perturbaciones biográficas a nivel familiar y profesional. El carácter duradero de la crisis y la falta de perspectivas temporales llevan a los individuos a reconsiderar su proyecto habitacional y a reajustar los horizontes temporales en materia de vivienda, según una lógica de anticipación de la crisis y de su temporalidad, con el fin de adaptarse a las contingencias e incertidumbres vinculadas al mercado inmobiliario. El análisis de las “turbulencias habitacionales” permite poner de manifiesto la gran diversidad de perfiles socioeconómicos de los individuos afectados por la crisis, sin dejar de lado las dinámicas inequitativas que estructuran la sociedad francesa. À partir des données de l’enquête La vie en confinement (Vico), cet article étudie ces perturbations en plaçant la focale sur les configurations résidentielles, entendues comme l’association entre un logement (l’espace qu’il procure, sa localisation, son environnement) et la composition du foyer (en considérant l’ensemble de ses occupants). Les données permettent de documenter le confinement du printemps 2020, ainsi que la suite de la crise jusqu’à la fin de l’année 2020. En combinant des descriptions statistiques et une analyse des commentaires libres rédigés par les personnes ayant répondu, nous montrons l’importance des « turbulences résidentielles » (changement de logement, accueil de personnes, départ de cohabitants) et leurs liens avec les perturbations biographiques sur les plans familial et professionnel. L’installation de la crise dans la durée et le manque de perspectives temporelles amènent également des individus à reconsidérer leur projet résidentiel et à réajuster les horizons temporels en matière de logement, selon une logique d’anticipation de la crise et de sa temporalité, pour s’adapter aux contingences et incertitudes liées au marché de l’immobilier. L’analyse des « turbulences résidentielles » permet de mettre à jour la grande diversité des profils socioéconomiques des individus touchés par la crise sans, toutefois, négliger les dynamiques inégalitaires qui structurent la société française, la crise venant renforcer des situations de vulnérabilité vécues par des individus faiblement dotés.

Le domaine de l'infectiologie constitue un champ de recherche transversal. Afin de l'identifier précisément, l'article construit un corpus global des publications en infectiologie en combinant les moyens offerts par le langage contrôlé du thésaurus MeSH de Medline et par la catégorisation des revues scientifiques du Web of Science. Ce corpus mondial permet de caractériser les publications des 20 premiers pays publiant dans le domaine et de retracer les évolutions entre 2000 à 2020. La construction de cartes thématiques permet d'identifier les thèmes de recherche au sein de l'infectiologie dans le monde et en France. L'explosion des publications sur le Covid-19 en 2020 a un impact très visible sur la carte mondiale des thématiques en infectiologie et modifie le positionnement de certains pays dans ce domaine de recherche. Research on infectious diseases constitutes a transversal scientific field. A specific corpus is designed by combining a controlled language (Medline MeSH thesaurus) and the categorization of journals (Web of Science). From this global corpus, the article characterizes the publications from the top 20 countries publishing in the field and evolutions between 2000 and 2020. Topic maps show the research themes within the field of infectious diseases both in the world and in France. The explosion of publications on Covid-19 in 2020 has a quite visible impact on the topic map in infectious diseases and changes the position of some countries in this field of research. The conclusion points to issues for further research as more complete data will become available on the Covid-19 period. International audience

Background: Coronavirus disease 2019 (COVID-19) emerged as a worldwide health crisis, overwhelming healthcare systems. Elevated cardiac troponin T (cTn T) at admission was associated with increased in-hospital mortality. However, data addressing the role of cTn T in major adverse cardiovascular events (MACE) in COVID-19 are scarce. Therefore, we assessed the role of baseline cTn T and cTn T kinetics for MACE and in-hospital mortality prediction in COVID-19. Methods: Three hundred and ten patients were included prospectively . One hundred and eight patients were excluded due to incomplete records. Patients were divided into three groups according to cTn T kinetics: ascending, descending, and constant. The cTn T slope was defined as the ratio of the cTn T change over time. The primary and secondary endpoints were MACE and in-hospital mortality. Results: Two hundred and two patients were included in the analysis (mean age 64.4 ± 16.7 years, 119 [58.9%] males). Mean duration of hospitalization was 14.0 ± 12.3 days. Sixty (29.7%) patients had MACE, and 40 (19.8%) patients died. Baseline cTn T predicted both endpoints ( p = 0.047, hazard ratio [HR] 1.805, 95% confidence interval [CI] 1.009 – 3.231 ; p = 0.009, HR 2.322, 95% CI 1.234 – 4.369) . Increased cTn T slope predicted mortality (p = 0.041, HR 1.006, 95% CI 1.000–1.011). Constant cTn T was associated with lower MACE and mortality (p = 0.000, HR 3.080, 95% CI 1.914–4.954, p = 0.000, HR 2.851, 95% CI 1.828–4.447). Conclusions: The present study emphasizes the additional role of cTn T testing in COVID-19 patients for risk stratification and improved diagnostic pathway and management.

Il s’appuie sur un questionnaire en ligne qui a recueilli 4 300 réponses pendant le printemps 2020. Nous interrogeons comment la séparation des espaces-temps due au confinement, caractérisé comme un temps d’incertitudes, de bouleversement des rythmes quotidiens et d’entrave aux relations en face à face, a influencé la fréquence des contacts. La pandémie a-t-elle resserré les liens ou au contraire les a-t-elle distendus avec les parents proches et éloignés ? La perturbation des rythmes quotidiens a-t-elle infléchi les contacts ? Le maintien de l’attention aux autres, notamment aux plus fragiles dans une parentèle élargie souvent peu étudiée, forme l’enjeu de cette analyse, autant que les inégalités de genre et de catégories sociales exacerbées par cette période. Globalement, la fréquence des contacts a augmenté, surtout avec la famille proche, mais aussi – et c’est l’originalité de cet article – avec des apparentés de 2e et 3e degrés.

Prior to the COVID-19 pandemic, a relative dearth of research existed in this domain regarding epidemics, which is surprising considering that this optimistic bias has been associated with a lack of engagement in protective behaviours critical in fighting twenty-first-century, emergent, infectious diseases. The current study addresses this gap in the literature by investigating whether people demonstrated optimism bias during the first wave of the COVID-19 pandemic in Europe, how this changed over time, and whether unrealistic optimism was negatively associated with protective measures. Taking advantage of a pre-existing international participative influenza surveillance network (n = 12,378), absolute and comparative unrealistic optimism were measured at three epidemic stages (pre-, early, peak), and across four countries—France, Italy, Switzerland and the United Kingdom. Despite differences in culture and health response, similar patterns were observed across all four countries. The prevalence of unrealistic optimism appears to be influenced by the particular epidemic context. Paradoxically, whereas absolute unrealistic optimism decreased over time, comparative unrealistic optimism increased, suggesting that whilst people became increasingly accurate in assessing their personal risk, they nonetheless overestimated that for others. Comparative unrealistic optimism was negatively associated with the adoption of protective behaviours, which is worrying, given that these preventive measures are critical in tackling the spread and health burden of COVID-19. It is hoped these findings will inspire further research into sociocognitive mechanisms involved in risk appraisal. International audience
