    BACKGROUND: Little is known about the practice of ventilation management in patients with COVID-19. We aimed to describe the practice of ventilation management and to establish outcomes in invasively ventilated patients with COVID-19 in a single country during the first month of the outbreak. METHODS: PRoVENT-COVID is a national, multicentre, retrospective observational study done at 18 intensive care units (ICUs) in the Netherlands. Consecutive patients aged at least 18 years were eligible for participation if they had received invasive ventilation for COVID-19 at a participating ICU during the first month of the national outbreak in the Netherlands. The primary outcome was a combination of ventilator variables and parameters over the first 4 calendar days of ventilation: tidal volume, positive end-expiratory pressure (PEEP), respiratory system compliance, and driving pressure. Secondary outcomes included the use of adjunctive treatments for refractory hypoxaemia and ICU complications. Patient-centred outcomes were ventilator-free days at day 28, duration of ventilation, duration of ICU and hospital stay, and mortality. PRoVENT-COVID is registered at ClinicalTrials.gov (NCT04346342). FINDINGS: Between March 1 and April 1, 2020, 553 patients were included in the study. Median tidal volume was 6·3 mL/kg predicted bodyweight (IQR 5·7-7·1), PEEP was 14·0 cm H2O (IQR 11·0-15·0), and driving pressure was 14·0 cm H2O (11·2-16·0). Median respiratory system compliance was 31·9 mL/cm H2O (26·0-39·9). Of the adjunctive treatments for refractory hypoxaemia, prone positioning was most often used in the first 4 days of ventilation (283 [53%] of 530 patients). The median number of ventilator-free days at day 28 was 0 (IQR 0-15); 186 (35%) of 530 patients had died by day 28. Predictors of 28-day mortality were gender, age, tidal volume, respiratory system compliance, arterial pH, and heart rate on the first day of invasive ventilation. INTERPRETATION: In patients with COVID-19 who were invasively ventilated during the first month of the outbreak in the Netherlands, lung-protective ventilation with low tidal volume and low driving pressure was broadly applied and prone positioning was often used. The applied PEEP varied widely, despite an invariably low respiratory system compliance. The findings of this national study provide a basis for new hypotheses and sample size calculations for future trials of invasive ventilation for COVID-19. These data could also help in the interpretation of findings from other studies of ventilation practice and outcomes in invasively ventilated patients with COVID-19.     Persistent ill health after acute COVID-19—referred to as long COVID, the post-acute COVID-19 syndrome, or the post-COVID-19 condition—has emerged as a major concern. We undertook an international consensus exercise to identify research priorities with the aim of understanding the long-term effects of acute COVID-19, with a focus on people with pre-existing airways disease and the occurrence of new-onset airways disease and associated symptoms. 202 international experts were invited to submit a minimum of three research ideas. After a two-phase internal review process, a final list of 98 research topics was scored by 48 experts. Patients with pre-existing or post-COVID-19 airways disease contributed to the exercise by weighting selected criteria. The highest-ranked research idea focused on investigation of the relationship between prognostic scores at hospital admission and morbidity at 3 months and 12 months after hospital discharge in patients with and without pre-existing airways disease. High priority was also assigned to comparisons of the prevalence and severity of post-COVID-19 fatigue, sarcopenia, anxiety, depression, and risk of future cardiovascular complications in patients with and without pre-existing airways disease. Our approach has enabled development of a set of priorities that could inform future research studies and funding decisions. This prioritisation process could also be adapted to other, non-respiratory aspects of long COVID.

    La popolazione deve essere omogeneamente distribuita sul territorio, attraverso azioni finalizzate al decentramento in alternativa alla densificazione urbana, in aree comunque già dotate di infrastrutture, o dove i nuovi collegamenti possano essere realizzati senza nuovo consumo di suolo ma con il recupero di aree già consumate (tenendo conto che circa l’80% del consumo di suolo dipende dalle infrastrutture). Al distanziamento sociale deve corrispondere il distanziamento urbanistico. Si auspicano, infine, azioni tese a dare priorità alla riqualificazione e rigenerazione delle INFRASTRUTTURE, includendo in esse anche le infrastrutture sociali, intese quelle afferenti al welfare in generale, e con particolare riferimento al sistema sanitario e scolastico. In estrema sintesi, la cosiddetta FASE 2 di ripartenza del Paese, non può prescindere dal rispetto dei tre capisaldi citati in apertura, assegnando priorità assoluta a tutte quelle attività proiettate nella direzione auspicata o che mettano in campo provvedimenti di adeguamento ad essa. Dal punto di vista della scala politica comunitaria forse accelerando il processo di unificazione delle nazioni europee nello Stato d’Europa ci consentirebbe di uniformare anche il corpo delle Leggi e delle Norme secondo il principio della unitarietà di governo. Il Governo dei Territori dovrà necessariamente andare verso una semplificazione che non deve essere intesa come rinuncia alla complessità e dovrà essere fondato su tre pilastri: 1) le Ragioni della Natura, come insieme delle caratteristiche intrinseche della natura dei luoghi e migliore destinazione; 2) le Ragioni della Storia, come insieme delle tracce delle stratificazioni delle azioni umane sul territorio e delle sue trasformazioni; 3) le Ragioni della Comunità, come insieme delle esigenze e dei fabbisogni delle popolazioni insediate di abitare, lavorare, muoversi e ricrearsi. Pochi strumenti chiari negli obiettivi da perseguire e nelle metodologie da attuare.

    La pandemia di Covid-19 ha generato significative implicazioni sociali, legali ed economiche. Nei Paesi oltre il Mediterraneo, segnati da un'instabilità finanziaria inarrestabile e da una continua crisi umanitaria, gli effetti dell'emergenza pandemica sembrano intaccare lo status già vulnerabile delle donne e le relazioni di genere, minacciando i fragili progressi compiuti in queste regioni. Infatti, le misure di isolamento sociale e distanziamento hanno avuto ripercussioni preoccupanti in termini di occupazione e accesso al lavoro, hanno esacerbato episodi di violenza domestica e reso più difficile l'accesso ai servizi essenziali, compresi quelli sanitari. Sebbene la pandemia di Covid-19 abbia ulteriormente evidenziato le vulnerabilità esistenti, si può notare che l'emergenza ha contribuito a mettere in mostra il ruolo principale e il potenziale delle donne. Le donne non solo rappresentano la maggior parte dei soccorritori in prima linea nelle emergenze nei settori sanitario, sociale, dell'insegnamento e dell'assistenza, ma sono state anche molto attive nella creazione di iniziative e imprese a sostegno della risposta alle crisi.

    Za razliku od kroničnih bolesnika i starijeg stanovništva, studentska populacija statistički ne spada u kategoriju medicinski najugroženije skupine od zaraze COVID-19. Stoga se postavlja pitanje kakvo je njihovo shvaćanje važećih epidemioloških mjere, posebno onih vezanih uz društveni život (fizičko distanciranje, socijalna izolacija, nastava na daljinu), te jesu li razvili alternativne načine izražavanja društvenosti? Kako bi pobliže istražili ova pitanja proveli smo kvalitativno istraživanje metodom fokus grupe sa studentima različitih fakulteta Sveučilišta u Zagrebu. Tematska analiza razgovora ukazuju na činjenicu da se studenti redovito pridržavaju mjera iz različitih altruističkih, ali i praktičnih razloga (strah da ne zaraze sebe, te drugog, strah od plaćanja kazne). Također, razvili su čitav niz novih oblika fizičkih dodira i gesta koji su prihvatljivi i u situaciji distanciranja (npr. pozdravi nogom). Ipak, i dalje naglašavaju važnost i nezamjenjivost interakcije licem u lice. Također, na temelju provedenog istraživanja postaju vidljivo da procjena rizika i opasnosti trenutne epidemije kod studenata ne mora nužno biti zdravstvene naravi, već je više usmjerena na moguće negativne ekonomske posljedice, te pad kvalitete obrazovanja.

    44-godišnjoj bolesnici s dijagnozom aksijalnog spondiloartritisa od 2018.g., koja je liječena NSAR u protuupalnoj dozi, zatim Salazopyrinom 2 gr dnevno, a od 22.8.2019. u terapiju je uveden adalimumab 40 mg 1x s.c. svakih 14 dana, dok nije uzimala glukokortikoide. Lijek je primjenjivan redovito, a na prvoj kontroli je bilo vidljivo subjektivno i kliničko poboljšanje. Od komorbiditeta bolesnica je imala cervikobrahijalni sindrom i arterijsku hipertenziju. Budući je bila u kontaktu s SARS-CoV-2 pozitivnom osobom 30.3.2020., testirana je i bila pozitivna (datum prvog testiranja 14.04.2020.), da bi u više navrata kroz naredna dva mjeseca bila pozitivna, a cijelo vrijeme bez ikakvih simptoma COVID-19 infekcije. Primjena adalimumaba prekinuta je nakon zadnje aplikacije (19.3.2020.). U razdoblju prekida primjene biološke terapije došlo je do pogoršanja spondiloartritisa u smislu pojačanja intenziteta bolova i trajanja zakočenosti dominantno u vratu i križima. Na reumatološkoj kontroli 23.6.2020. priloženi su testovi od 9.6.2020. i 11.6.2020. CoV RdRp gen neg, CoV R gen neg, CoV gen neg., te je u terapiju ponovno uveden adalimumab. U kliničkom aspektu SARS-CoV-2 infekcija varira od asimptomatske do fatalne bolesti. Oko 80% labaratorijski potvrđenih COVID- 19 bolesnika imaju blagu do umjerenu kliničku sliku, 13, 8% ozbiljnu kliničku sliku, a 6, 1% kritičnu bolest koja zahtjeva hospitalizaciju u jedinicu intenzivne njege. Razni faktori povezani su s lošom prognozom uključujući: stariju dob, muški spol i komorbiditete dok imunološke bolesti i imunosupresivna terapija povećavaju prijemčivost infektivnim bolestima. Također, postoji povećana prevalencija hospitalizacije bolesnika s bolestima vezivnog tkiva i COVID-19 i bolesnika liječenih ts/bDMARD u usporedbi s referentnom grupom. Za bolesnike s upalnim reumatskim bolestima upotreba glukokortikoida (GK) povezana je s hospitalizacijom i lošijim ishodom, dok to nije dokazano za tradicionalne imunosupresive, csDMARD niti za ts/ bDMARD-e. EULAR i Hrvatsko reumatološka društva preporučuju da bolesnici s upalnim reumatskim bolestima koji su na imunosupresivnoj terapiji ne prekidaju GK tijekom COVID-19 infekcije, iako se sugerira upotreba minimalne potrebne doze. Također, treba nastaviti s csDMARD i ts/bDMARD, ovisno o kliničkoj slici. Problemu prekida ili nastavka liječenja tim lijekovima treba pristupiti individualno.

    The anthraquinones are a huge group of natural compounds, and one of the compounds from that group is 1, 8-dihydroxy-3-methoxy-6-methyl- anthracene-9, 10-dione (parietin, known as physcion as well). The parietin is produced by various plants such as R. crispus and R. japonicus Houtt., as well as many fungi such as Aspergillus spp., Penicillium spp., and Alternaria spp. It is well known that parietin possesses antitumor activity, antioxidant activity, antimutagenic activity, topoisomerase II inhibitory activity, in vitro antiviral activity against poliovirus, and antifungal activity. Knowing these facts it can be supposed that they can found application in medicinal chemistry. A study was conducted to explore the antioxidative and inhibitory potency of parietin. Density functional theory (DFT) is a powerful tool used for examining the antioxidative mechanisms. The Gaussian 09 program package is used to perform all electronic calculations. The equilibrium geometries of the parent molecule of parietin, its radical cation, radicals, and anions are calculated using the B3LYP-D3BJ functional in conjunction with the 6-311++G(d, p) basis set. The influence of water as a polar solvent (ε = 78.36) was estimated using the CPCM solvation model. Inhibitory potency of parientin is estimated by applying molecular docking simulations. For this purpose AutoDock 4.0 software is used, and inhibitory potency of parietin is estimated toward main protease Mpro from SARS-CoV-2.

    BACKGROUND: Little is known about the practice of ventilation management in patients with COVID-19. We aimed to describe the practice of ventilation management and to establish outcomes in invasively ventilated patients with COVID-19 in a single country during the first month of the outbreak. METHODS: PRoVENT-COVID is a national, multicentre, retrospective observational study done at 18 intensive care units (ICUs) in the Netherlands. Consecutive patients aged at least 18 years were eligible for participation if they had received invasive ventilation for COVID-19 at a participating ICU during the first month of the national outbreak in the Netherlands. The primary outcome was a combination of ventilator variables and parameters over the first 4 calendar days of ventilation: tidal volume, positive end-expiratory pressure (PEEP), respiratory system compliance, and driving pressure. Secondary outcomes included the use of adjunctive treatments for refractory hypoxaemia and ICU complications. Patient-centred outcomes were ventilator-free days at day 28, duration of ventilation, duration of ICU and hospital stay, and mortality. PRoVENT-COVID is registered at ClinicalTrials.gov (NCT04346342). FINDINGS: Between March 1 and April 1, 2020, 553 patients were included in the study. Median tidal volume was 6·3 mL/kg predicted bodyweight (IQR 5·7-7·1), PEEP was 14·0 cm H2O (IQR 11·0-15·0), and driving pressure was 14·0 cm H2O (11·2-16·0). Median respiratory system compliance was 31·9 mL/cm H2O (26·0-39·9). Of the adjunctive treatments for refractory hypoxaemia, prone positioning was most often used in the first 4 days of ventilation (283 [53%] of 530 patients). The median number of ventilator-free days at day 28 was 0 (IQR 0-15); 186 (35%) of 530 patients had died by day 28. Predictors of 28-day mortality were gender, age, tidal volume, respiratory system compliance, arterial pH, and heart rate on the first day of invasive ventilation. INTERPRETATION: In patients with COVID-19 who were invasively ventilated during the first month of the outbreak in the Netherlands, lung-protective ventilation with low tidal volume and low driving pressure was broadly applied and prone positioning was often used. The applied PEEP varied widely, despite an invariably low respiratory system compliance. The findings of this national study provide a basis for new hypotheses and sample size calculations for future trials of invasive ventilation for COVID-19. These data could also help in the interpretation of findings from other studies of ventilation practice and outcomes in invasively ventilated patients with COVID-19.     Persistent ill health after acute COVID-19—referred to as long COVID, the post-acute COVID-19 syndrome, or the post-COVID-19 condition—has emerged as a major concern. We undertook an international consensus exercise to identify research priorities with the aim of understanding the long-term effects of acute COVID-19, with a focus on people with pre-existing airways disease and the occurrence of new-onset airways disease and associated symptoms. 202 international experts were invited to submit a minimum of three research ideas. After a two-phase internal review process, a final list of 98 research topics was scored by 48 experts. Patients with pre-existing or post-COVID-19 airways disease contributed to the exercise by weighting selected criteria. The highest-ranked research idea focused on investigation of the relationship between prognostic scores at hospital admission and morbidity at 3 months and 12 months after hospital discharge in patients with and without pre-existing airways disease. High priority was also assigned to comparisons of the prevalence and severity of post-COVID-19 fatigue, sarcopenia, anxiety, depression, and risk of future cardiovascular complications in patients with and without pre-existing airways disease. Our approach has enabled development of a set of priorities that could inform future research studies and funding decisions. This prioritisation process could also be adapted to other, non-respiratory aspects of long COVID.

    La popolazione deve essere omogeneamente distribuita sul territorio, attraverso azioni finalizzate al decentramento in alternativa alla densificazione urbana, in aree comunque già dotate di infrastrutture, o dove i nuovi collegamenti possano essere realizzati senza nuovo consumo di suolo ma con il recupero di aree già consumate (tenendo conto che circa l’80% del consumo di suolo dipende dalle infrastrutture). Al distanziamento sociale deve corrispondere il distanziamento urbanistico. Si auspicano, infine, azioni tese a dare priorità alla riqualificazione e rigenerazione delle INFRASTRUTTURE, includendo in esse anche le infrastrutture sociali, intese quelle afferenti al welfare in generale, e con particolare riferimento al sistema sanitario e scolastico. In estrema sintesi, la cosiddetta FASE 2 di ripartenza del Paese, non può prescindere dal rispetto dei tre capisaldi citati in apertura, assegnando priorità assoluta a tutte quelle attività proiettate nella direzione auspicata o che mettano in campo provvedimenti di adeguamento ad essa. Dal punto di vista della scala politica comunitaria forse accelerando il processo di unificazione delle nazioni europee nello Stato d’Europa ci consentirebbe di uniformare anche il corpo delle Leggi e delle Norme secondo il principio della unitarietà di governo. Il Governo dei Territori dovrà necessariamente andare verso una semplificazione che non deve essere intesa come rinuncia alla complessità e dovrà essere fondato su tre pilastri: 1) le Ragioni della Natura, come insieme delle caratteristiche intrinseche della natura dei luoghi e migliore destinazione; 2) le Ragioni della Storia, come insieme delle tracce delle stratificazioni delle azioni umane sul territorio e delle sue trasformazioni; 3) le Ragioni della Comunità, come insieme delle esigenze e dei fabbisogni delle popolazioni insediate di abitare, lavorare, muoversi e ricrearsi. Pochi strumenti chiari negli obiettivi da perseguire e nelle metodologie da attuare.

    La pandemia di Covid-19 ha generato significative implicazioni sociali, legali ed economiche. Nei Paesi oltre il Mediterraneo, segnati da un'instabilità finanziaria inarrestabile e da una continua crisi umanitaria, gli effetti dell'emergenza pandemica sembrano intaccare lo status già vulnerabile delle donne e le relazioni di genere, minacciando i fragili progressi compiuti in queste regioni. Infatti, le misure di isolamento sociale e distanziamento hanno avuto ripercussioni preoccupanti in termini di occupazione e accesso al lavoro, hanno esacerbato episodi di violenza domestica e reso più difficile l'accesso ai servizi essenziali, compresi quelli sanitari. Sebbene la pandemia di Covid-19 abbia ulteriormente evidenziato le vulnerabilità esistenti, si può notare che l'emergenza ha contribuito a mettere in mostra il ruolo principale e il potenziale delle donne. Le donne non solo rappresentano la maggior parte dei soccorritori in prima linea nelle emergenze nei settori sanitario, sociale, dell'insegnamento e dell'assistenza, ma sono state anche molto attive nella creazione di iniziative e imprese a sostegno della risposta alle crisi.
