    In maart 2020 moesten gerechtsgebouwen ruim twee maanden sluiten vanwege het coronavirus. Zaken werden schriftelijk afgedaan en zittingen voor urgente zaken vonden telefonisch of online plaats. Het project ‘De impact van de coronacrisis op de rechtspraak en de positie van kwetsbare rechtszoekenden’ onderzocht de gevolgen hiervan voor de rechtspraak en kwetsbare rechtzoekenden in het strafrecht, civiele jeugdrecht en vreemdelingenrecht. Data werden verzameld door middel van interviews met rechters, advocaten en andere professionals, enquêtes onder rechtzoekenden en een grondrechtenanalyse. Het onderzoek werd uitgevoerd door onderzoekers van de rechtenfaculteiten van de universiteiten van Leiden, Utrecht en Nijmegen, met een subsidie van ZonMW. Looptijd: september 2020 – maart 2022. Deze niet-openbare data collection bevat de dataset van het onderzoek. De data van de survey onder gedetineerden worden voor hergebruik beschikbaar gemaakt via het DANS archiveringssysteem.

    BACKGROUND: Little is known about the practice of ventilation management in patients with COVID-19. We aimed to describe the practice of ventilation management and to establish outcomes in invasively ventilated patients with COVID-19 in a single country during the first month of the outbreak. METHODS: PRoVENT-COVID is a national, multicentre, retrospective observational study done at 18 intensive care units (ICUs) in the Netherlands. Consecutive patients aged at least 18 years were eligible for participation if they had received invasive ventilation for COVID-19 at a participating ICU during the first month of the national outbreak in the Netherlands. The primary outcome was a combination of ventilator variables and parameters over the first 4 calendar days of ventilation: tidal volume, positive end-expiratory pressure (PEEP), respiratory system compliance, and driving pressure. Secondary outcomes included the use of adjunctive treatments for refractory hypoxaemia and ICU complications. Patient-centred outcomes were ventilator-free days at day 28, duration of ventilation, duration of ICU and hospital stay, and mortality. PRoVENT-COVID is registered at ClinicalTrials.gov (NCT04346342). FINDINGS: Between March 1 and April 1, 2020, 553 patients were included in the study. Median tidal volume was 6·3 mL/kg predicted bodyweight (IQR 5·7-7·1), PEEP was 14·0 cm H2O (IQR 11·0-15·0), and driving pressure was 14·0 cm H2O (11·2-16·0). Median respiratory system compliance was 31·9 mL/cm H2O (26·0-39·9). Of the adjunctive treatments for refractory hypoxaemia, prone positioning was most often used in the first 4 days of ventilation (283 [53%] of 530 patients). The median number of ventilator-free days at day 28 was 0 (IQR 0-15); 186 (35%) of 530 patients had died by day 28. Predictors of 28-day mortality were gender, age, tidal volume, respiratory system compliance, arterial pH, and heart rate on the first day of invasive ventilation. INTERPRETATION: In patients with COVID-19 who were invasively ventilated during the first month of the outbreak in the Netherlands, lung-protective ventilation with low tidal volume and low driving pressure was broadly applied and prone positioning was often used. The applied PEEP varied widely, despite an invariably low respiratory system compliance. The findings of this national study provide a basis for new hypotheses and sample size calculations for future trials of invasive ventilation for COVID-19. These data could also help in the interpretation of findings from other studies of ventilation practice and outcomes in invasively ventilated patients with COVID-19.     Persistent ill health after acute COVID-19—referred to as long COVID, the post-acute COVID-19 syndrome, or the post-COVID-19 condition—has emerged as a major concern. We undertook an international consensus exercise to identify research priorities with the aim of understanding the long-term effects of acute COVID-19, with a focus on people with pre-existing airways disease and the occurrence of new-onset airways disease and associated symptoms. 202 international experts were invited to submit a minimum of three research ideas. After a two-phase internal review process, a final list of 98 research topics was scored by 48 experts. Patients with pre-existing or post-COVID-19 airways disease contributed to the exercise by weighting selected criteria. The highest-ranked research idea focused on investigation of the relationship between prognostic scores at hospital admission and morbidity at 3 months and 12 months after hospital discharge in patients with and without pre-existing airways disease. High priority was also assigned to comparisons of the prevalence and severity of post-COVID-19 fatigue, sarcopenia, anxiety, depression, and risk of future cardiovascular complications in patients with and without pre-existing airways disease. Our approach has enabled development of a set of priorities that could inform future research studies and funding decisions. This prioritisation process could also be adapted to other, non-respiratory aspects of long COVID.

    La popolazione deve essere omogeneamente distribuita sul territorio, attraverso azioni finalizzate al decentramento in alternativa alla densificazione urbana, in aree comunque già dotate di infrastrutture, o dove i nuovi collegamenti possano essere realizzati senza nuovo consumo di suolo ma con il recupero di aree già consumate (tenendo conto che circa l’80% del consumo di suolo dipende dalle infrastrutture). Al distanziamento sociale deve corrispondere il distanziamento urbanistico. Si auspicano, infine, azioni tese a dare priorità alla riqualificazione e rigenerazione delle INFRASTRUTTURE, includendo in esse anche le infrastrutture sociali, intese quelle afferenti al welfare in generale, e con particolare riferimento al sistema sanitario e scolastico. In estrema sintesi, la cosiddetta FASE 2 di ripartenza del Paese, non può prescindere dal rispetto dei tre capisaldi citati in apertura, assegnando priorità assoluta a tutte quelle attività proiettate nella direzione auspicata o che mettano in campo provvedimenti di adeguamento ad essa. Dal punto di vista della scala politica comunitaria forse accelerando il processo di unificazione delle nazioni europee nello Stato d’Europa ci consentirebbe di uniformare anche il corpo delle Leggi e delle Norme secondo il principio della unitarietà di governo. Il Governo dei Territori dovrà necessariamente andare verso una semplificazione che non deve essere intesa come rinuncia alla complessità e dovrà essere fondato su tre pilastri: 1) le Ragioni della Natura, come insieme delle caratteristiche intrinseche della natura dei luoghi e migliore destinazione; 2) le Ragioni della Storia, come insieme delle tracce delle stratificazioni delle azioni umane sul territorio e delle sue trasformazioni; 3) le Ragioni della Comunità, come insieme delle esigenze e dei fabbisogni delle popolazioni insediate di abitare, lavorare, muoversi e ricrearsi. Pochi strumenti chiari negli obiettivi da perseguire e nelle metodologie da attuare.

    La pandemia di Covid-19 ha generato significative implicazioni sociali, legali ed economiche. Nei Paesi oltre il Mediterraneo, segnati da un'instabilità finanziaria inarrestabile e da una continua crisi umanitaria, gli effetti dell'emergenza pandemica sembrano intaccare lo status già vulnerabile delle donne e le relazioni di genere, minacciando i fragili progressi compiuti in queste regioni. Infatti, le misure di isolamento sociale e distanziamento hanno avuto ripercussioni preoccupanti in termini di occupazione e accesso al lavoro, hanno esacerbato episodi di violenza domestica e reso più difficile l'accesso ai servizi essenziali, compresi quelli sanitari. Sebbene la pandemia di Covid-19 abbia ulteriormente evidenziato le vulnerabilità esistenti, si può notare che l'emergenza ha contribuito a mettere in mostra il ruolo principale e il potenziale delle donne. Le donne non solo rappresentano la maggior parte dei soccorritori in prima linea nelle emergenze nei settori sanitario, sociale, dell'insegnamento e dell'assistenza, ma sono state anche molto attive nella creazione di iniziative e imprese a sostegno della risposta alle crisi.

    In maart 2020 moesten gerechtsgebouwen ruim twee maanden sluiten vanwege het coronavirus. Zaken werden schriftelijk afgedaan en zittingen voor urgente zaken vonden telefonisch of online plaats. Het project ‘De impact van de coronacrisis op de rechtspraak en de positie van kwetsbare rechtszoekenden’ onderzocht de gevolgen hiervan voor de rechtspraak en kwetsbare rechtzoekenden in het strafrecht, civiele jeugdrecht en vreemdelingenrecht. Data werden verzameld door middel van interviews met rechters, advocaten en andere professionals, enquêtes onder rechtzoekenden en een grondrechtenanalyse. Het onderzoek werd uitgevoerd door onderzoekers van de rechtenfaculteiten van de universiteiten van Leiden, Utrecht en Nijmegen, met een subsidie van ZonMW. Looptijd: september 2020 – maart 2022. Deze niet-openbare data collection bevat de dataset van het onderzoek. De data van de survey onder gedetineerden worden voor hergebruik beschikbaar gemaakt via het DANS archiveringssysteem.
